Governance Explanations

Form 990, Part VI, Section B, Line 11B

The finance committee reviews and approves the form 990. A copy of the completed 990 is provided to each member of the board of directors prior to signing and filing the return.

Form 990, Part VI, Section B, Line 12C

Every year, officers, directors, non-board committee members, and key employees are required to sign an affirmation of compliance with hawc's conflict of interest policy and complete a disclosure statement about any relationships that constitute conflicts of interest. In addition, officers, directors, non-board committee members, and key employees are required to report during the year any relationships that are potential conflicts that arise. Any such relationships between a board member or the executive director and hawc are reviewed and approved by the board,any such relationships between staff and hawc are reviewed by the executive director.

Form 990, Part VI, Section B, Line 15

The executive committee of the board solicits input from all board members on executive director performance annually, and also reviews an annual self-evaluation by the executive director. Bi-annually, the executive committee solicits staff input. Input is sought relating to five general areas of performance, progress toward meeting the executive director's established goals, and the executive director's relationship with the board. The executive committee reviews comparable compensation data periodically. Based on all input, the exectuive committee creates a preliminary report, which it reviews with the executive director. The executive committee then submits to the board report on executive director performance and a salary recommendation. The final salary determination, and approval, comment or rejection of the review report is made by the board. Hawc does not have a ceo, the board president is not compensated. Compensation for other top management employees is determined by the executive director, subject to board approval as part of the annual budget process (but not on an employee by employee basis)

EACH OF THE COMMUNITY BASED OFFICES ARE CO-LOCATED WITH COMPLIMENTARY HUMAN SERVICE PROVIDERS SUCH AS THE DEPARTMENT OF TRANSITIONAL ASSISTANCE, WIC, COMMUNITY HEALTH CENTERS AND CAREER COUNSELING.

GROUP SUPPORT INCLUDES DOMESTIC VIOLENCE 101, PARENTING SUPPORT AND CREATIVE COPING, A PARENT CHILD SUPPORT GROUP THAT USES ART, DANCE, AND MOVEMENT TO HELP KIDS LABEL AND COMMUNICATE THEIR EMOTIONS AND ENGAGE COPING SKILLS.
