Form 990 is prepared by our audit firm based on information provided by health imperatives' management. The chief financial officer reviews the completed form 990 for accuracy, and any corrections or changes are agreed upon and made by the audit firm. A final review of the form 990 is then done by the chief executive officer, the treasurer of the board of directors, and then by the full board.

Form 990, Part VI, Section B, Line 12C

All of health imperatives' board members are required to sign the conflict of interest form annually. Management tracks form signing to insure full compliance.

To determine the appropriate compensation for the chief executive officer and other key employees a salary comparative analysis is conducted by an outside service in conjunction with health imperatives. Additionally, the hr department does a comparative analysis and evaluations are done for all staff.

Health imperatives' mission is to improve the health and well-being of low-income or vulnerable families and individuals in southeastern massachusetts. We aim to meet the diverse needs of a growing population of underserved individuals with complex needs in communities stretching from quincy to nantucket. The majority of those we serve have multiple barriers to good health, safety, and economic security. We are committed to creating innovative ways of reaching people who are particularly at-risk, including survivors of interpersonal violence, immigrants who are afraid to seek health care or emergency support, people with disabilities, lgbtq individuals, people who are homeless, and individuals with substance use disorders. Over the past few years we have restructured our services, focused on our core competencies, and developed strategic alliances in order to fulfill our mission. We believe that it is essential to streamline and coordinate services within the organization and with others in our region, particularly given declining resources and increasing needs.

The audit committee appointed by the board of directors assumes responsibility for the oversight of the audit of its financial statements and the selection of the independent accountant.

PART X, LINE 2:

The agency accounts for uncertainty in income taxes in accordance with asc topic, income taxes. This standard clarifies the accounting for uncertainty in tax positions and prescribes a recognition threshold and measurement attribute for the financial statements regarding a tax position taken or expected to be taken in a tax return. The agency has determined that there are no uncertain tax positions which qualify for either recognition or disclosure in the financial statements at june 30, 2021. The agency's information returns are subject to examination by the federal and state jurisdictions.

WE ALSO PROVIDE WORKSHOPS, TRAINING, AND TECHNICAL ASSISTANCE TO EDUCATE INDIVIDUALS ABOUT INTERPERSONAL VIOLENCE AND TO ENGAGE COMMUNITIES IN THE EFFORT TO PREVENT VIOLENCE.
