“Kids4health institute, a health, fitness and wellness services organization, served a total of 128 youth with 90 youth completing one or more 10-12 week programs at our two sites, with over 10 parents participating alongside their children, respectively. Our program and services assist children and families address obesity, and weight management challenges in partnership with their physicians to prevent and manage acute and chronic diseases such as diabetes, cardio-vascular diseases, and hyper tension. Seventy percent of our clients were referred by their physicians. K4h core services includ health and weight management workshops, counseling, nutrition and diet workshops and intense fitness activities. We have witnessed our clients significantly improving their overall health with many making major progress towards reversing the impact of pre-diabetes and hypertensions acute conditions. In several instances, participants with pre-existing health conditions such as pre diabetic and high blood pressure have made major progress towards reversing the impact of their acute diseases and healing their bodies.”
“Community impact - in collaboration with the brooklyn school district and high school yds launched a program as a part of a broader community effort to address not only obesity but also the growing health disparities in the community. The program was offered to. Yds provided students during the extended academic hour the opportunity to participate in nutrition workshops and fitness activities. We served 32 students with 40% considered overweight or obese, with a total of 18 students periodically attending workout sessions and nutrition and diet workshops. As a member of the brooklyn center healthy living hub collaborative, we collaborated with school district, and other agencies to improve the overall health and wellness of students and residents. As a part of our partnership, we held our move more eat better community fitness and health fair that included over 18 community health organization exhibitors, and over 1,100 people attended and participated in a host of activities. Additionally, we have supported and participated in other health events in association with other community, faith-base and health organizations. Yds's track mn elite program included 73 participants and 21 parents attending 8 nutrition and diet workshops. Additionally, the parents engaged in a walk/run and workout group during the track season from may to august, 2013. The participants in the track program, in addition to getting healthier, competed at the highest level of aau track and field with many achieving academic and all american status. Ninety percent of members graduating from high school receive academic and athletic scholarships.”

OUR PROGRAM AND SERVICES ASSIST CHILDREN AND FAMILIES ADDRESS OBESITY, AND WEIGHT MANAGEMENT CHALLENGES IN PARTNERSHIP WITH THEIR PHYSICIANS TO PREVENT AND MANAGE ACUTE AND CHRONIC DISEASES SUCH AS DIABETES, CARDIO-VASCULAR DISEASES, AND HYPER TENSION. SEVENTY PERCENT OF OUR CLIENTS WERE REFERRED BY THEIR PHYSICIANS. K4H CORE SERVICES INCLUD HEALTH AND WEIGHT MANAGEMENT WORKSHOPS, COUNSELING, NUTRITION AND DIET WORKSHOPS AND INTENSE FITNESS ACTIVITIES. WE HAVE WITNESSED OUR CLIENTS SIGNIFICANTLY IMPROVING THEIR OVERALL HEALTH WITH MANY MAKING MAJOR PROGRESS TOWARDS REVERSING THE IMPACT OF PRE-DIABETES AND HYPERTENSIONS ACUTE CONDITIONS. IN SEVERAL INSTANCES, PARTICIPANTS WITH PRE-EXISTING HEALTH CONDITIONS SUCH AS PRE DIABETIC AND HIGH BLOOD PRESSURE HAVE MADE MAJOR PROGRESS TOWARDS REVERSING THE IMPACT OF THEIR ACUTE DISEASES AND HEALING THEIR BODIES. |

WE SERVED 32 STUDENTS WITH 40% CONSIDERED OVERWEIGHT OR OBESE, WITH A TOTAL OF 18 STUDENTS PERIODICALLY ATTENDING WORKOUT SESSIONS AND NUTRITION AND DIET WORKSHOPS. AS A MEMBER OF THE BROOKLYN CENTER HEALTHY LIVING HUB COLLABORATIVE, WE COLLABORATED WITH SCHOOL DISTRICT, AND OTHER AGENCIES TO IMPROVE THE OVERALL HEALTH AND WELLNESS OF STUDENTS AND RESIDENTS. AS A PART OF OUR PARTNERSHIP, WE HELD OUR MOVE MORE EAT BETTER COMMUNITY FITNESS AND HEALTH FAIR THAT INCLUDED OVER 18 COMMUNITY HEALTH ORGANIZATION EXHIBITORS, AND OVER 1,100 PEOPLE ATTENDED AND PARTICIPATED IN A HOST OF ACTIVITIES. ADDITIONALLY, WE HAVE SUPPORTED AND PARTICIPATED IN OTHER HEALTH EVENTS IN ASSOCIATION WITH OTHER COMMUNITY, FAITH-BASE AND HEALTH ORGANIZATIONS. YDS'S TRACK MN ELITE PROGRAM INCLUDED 73 PARTICIPANTS AND 21 PARENTS ATTENDING 8 NUTRITION AND DIET WORKSHOPS. ADDITIONALLY, THE PARENTS ENGAGED IN A WALK/RUN AND WORKOUT GROUP DURING THE TRACK SEASON FROM MAY TO AUGUST, 2013. THE PARTICIPANTS IN THE TRACK PROGRAM, IN ADDITION TO GETTING HEALTHIER, COMPETED AT THE HIGHEST LEVEL OF AAU TRACK AND FIELD WITH MANY ACHIEVING ACADEMIC AND ALL AMERICAN STATUS. NINETY PERCENT OF MEMBERS GRADUATING FROM HIGH SCHOOL RECEIVE ACADEMIC AND ATHLETIC SCHOLARSHIPS. |
