Last year we performed ongoing maintenance, repairs, and usability improvements, such as installing programmable thermostats, continuing the transition to LED bulbs,and installing a floor-level scale for patient weigh-in. The corporation was dissolved on June 15, 2022. All assets of the corporation, including the building,land, and liquid assets, were distributed to or are in the course of being distributed to HealthReach. |

The health center is a Federally Qualified Health Center and has been in operation at these premises, with the corporation as landlord, since 1980.On April 20, 2022 the members of the corporation approved a plan of dissolution pursuant to which all of the corporations net assets would be donated to HealthReach for the continued operation of the health center. This was accomplished via two cash distributions and the transfer of real property listed in Part II of this form. The corporation filed Articles of Dissolution with the Maine Secretary of State on June 15, 2022.The second of the two cash distributions occurred subsequent to the tax year for which this return is filed.Question 2(a)At the time of dissolution John Opperman was a director of the corporation and president of the corporations board of directors. Mr. Opperman served as director continuously from the 2013-14 period to the time of dissolution and served as board president continuously from the 2016-17 period to the time of dissolution.Mr. Opperman also served as a director of HealthReach from January 2014 to January 2020.
